Eligibility
Program Description
Alpha Life provides a virtual Hormone Optimization program, including testosterone therapy (TRT) where clinically appropriate.
The program includes clinical assessment, laboratory review, consultation, and ongoing monitoring by a licensed Nurse Practitioner.
As part of the Founding Member Program, the first 100 eligible and clinically approved individuals may receive Founding Member pricing for the Hormone Optimization program, subject to the terms outlined below.
Participation in the program does not guarantee eligibility for treatment or prescription, which remains subject to clinical judgment.
Founding Member benefits apply exclusively to the Hormone Optimization (TRT) program and do not apply to other services offered by Alpha Life.
Enrollment Process
To be considered for the Hormone Optimization program, individuals must:
- Submit their information through Alpha Life’s intake process
- Complete all required intake steps through Alpha Life’s secure clinical platform
- Undergo clinical review, including laboratory testing where applicable, by a licensed Nurse Practitioner
All applicants must be medically approved before beginning treatment or becoming Founding Members.
Founding Member Limit
Founding Member pricing is limited to the first 100 individuals who are clinically approved and enrolled in the Hormone Optimization program.
Limit one Founding Member enrollment per person.
Subscription Activation
Approved Founding Members must activate their Hormone Optimization subscription within 90 days of approval.
If a Founding Member does not activate their subscription within this period, the Founding Member offer and associated pricing will expire and cannot be reinstated.
Founding Member Pricing
Approved Founding Members who activate their subscription within the required period will receive Hormone Optimization pricing of $33/month (regular price $66/month) for as long as the member maintains an active subscription.
If a member cancels their subscription at any time, Founding Member pricing will be permanently forfeited and cannot be reinstated.
Founding Member pricing applies only to the registered individual and may not be transferred.
Fees & Payment
Fees associated with the Hormone Optimization program may include clinical assessment, laboratory review, consultation, ongoing care, and subscription billing.
By enrolling in the program, you agree to applicable fees and billing practices as communicated at the time of purchase or enrollment.
Non-Transferable
Founding Member pricing applies only to the registered individual and cannot be transferred.
Subscription & Billing
The Hormone Optimization program may be offered on a recurring subscription basis.
By enrolling in a subscription, you agree to ongoing billing at the stated interval unless cancelled.
You may cancel your subscription at any time, and cancellation will take effect at the end of the current billing cycle.
Alpha Life reserves the right to modify standard program pricing with reasonable notice. This does not apply to active Founding Members maintaining uninterrupted eligibility under the Founding Member Program.
Fees & Refunds
Fees paid to initiate the Hormone Optimization program are for clinical assessment, laboratory review, consultation, and program onboarding.
If a licensed Nurse Practitioner determines that treatment is not clinically appropriate following review of laboratory results or consultation, any fees paid will be fully refunded.
If you choose not to proceed with treatment after your laboratory results have been reviewed, a clinical review fee of $60 may be retained to cover the cost of laboratory analysis and Nurse Practitioner time. Any remaining balance will be refunded.
Once treatment has been initiated or a prescription has been issued, services are considered to have commenced and fees are non-refundable.
Subscription fees, once activated, are non-refundable.
Patient Responsibilities
You agree to:
- Provide accurate, complete, and truthful information
- Complete required laboratory testing and follow-up assessments
- Follow all treatment instructions provided
- Report any side effects or concerns
- Seek appropriate medical care when necessary
Failure to comply with program requirements may result in modification, suspension, or discontinuation of treatment.
Clinical Discretion
All treatment decisions are made by a licensed Nurse Practitioner based on clinical judgment.
The Nurse Practitioner may:
- Request additional information
- Recommend alternative care
- Adjust or discontinue treatment
- Decline treatment if deemed clinically appropriate
Service Continuity
Alpha Life reserves the right to modify, update, or discontinue specific services, treatment protocols, or program structures as medical standards, regulations, or operational requirements evolve.
If the Hormone Optimization program is materially changed or replaced by a comparable service, active Founding Members will retain their Founding Member pricing for the closest equivalent service offering, provided their subscription remains active and uninterrupted.
Where possible, reasonable notice will be provided.
Privacy
Personal information is collected and handled in accordance with the Alpha Life Privacy Policy.
Acceptance
By enrolling in the Founding Member Hormone Optimization program, you confirm that you have read, understood, and agree to these Terms & Conditions.
